Free and Low-Cost Addiction Treatment in New York City

Finding free or low-cost addiction treatment in New York City means navigating five very different boroughs, each with its own mix of public hospitals, nonprofits, and harm reduction organizations. Detox, outpatient counseling, medication for opioid use disorder, and syringe services are available at no cost or on a sliding scale somewhere in the city, regardless of insurance status.

Manhattan has the city’s deepest public investment and is home to OnPoint NYC, which opened the nation’s first two publicly recognized overdose prevention centers in East Harlem and Washington Heights, alongside a Lower East Side syringe exchange and a nonprofit outpatient program offering methadone and buprenorphine since 1960. The Bronx carries the city’s highest overdose rate, but also two of its most active harm reduction organizations, St. Ann’s Corner of Harm Reduction and BOOM!Health, both offering free naloxone, syringes, and low-threshold buprenorphine from the South Bronx.

Queens runs NYC Health + Hospitals/Elmhurst, a public hospital treating people regardless of ability to pay, plus AIDS Center of Queens County’s harm reduction sites and a dense network of AA, NA, and Al-Anon meetings. Brooklyn offers NYC Health + Hospitals/South Brooklyn Health’s outpatient medication program, along with The After Hours Project and VOCAL-NY, two nonprofits running free syringe and naloxone services, including a 24-hour vending machine in southern Brooklyn.

Staten Island’s dedicated page is still being built out. In the meantime, free naloxone is available by mail statewide, and 211 or the state’s HOPEline can help match any New York City resident to a program in whichever borough has room.

Free Addiction Treatment Resources in New York City

Free Treatment Score: 6.3 / 10. This score is anchored on New York County (Manhattan) data, the borough with the deepest publicly funded treatment infrastructure in the city; each borough below has its own score and its own local resources.

  • Treatment availability: Strong. New York City runs some of the largest publicly funded and sliding-scale behavioral health systems in the country, anchored by NYC Health + Hospitals and a network of state-certified providers in every borough.
  • Coverage and legal support: Strong. New York expanded Medicaid, and the state’s naloxone standing order and legal syringe services apply citywide, in every borough without exception.
  • Local cost of care: Higher than average. New York City runs well above the national average on services costs, though the free and sliding-scale paths below do not depend on local prices.
  • Harm reduction access: Active. The city is home to the nation’s first two overdose prevention centers plus syringe service programs in the Bronx, Queens, and Brooklyn, giving naloxone, testing, and supervised use real citywide reach.

New York City’s free and low-cost safety net is one of the largest and most varied in the country, though what is actually reachable depends heavily on which borough someone is standing in. This page pulls together citywide resources and draws its score from New York County (Manhattan) data; each borough has its own dedicated page with a local score and its own resources, linked from the editorial below.

Across all five boroughs, Medicaid rehab options in New York cover the bulk of what residents need, and city and state hotlines can help route around a program that is full. Harm reduction, from mailed naloxone to the city’s overdose prevention centers, does not require insurance or a referral anywhere in New York City.
