Case managers minimize these problems by working alongside you to identify where you need extra support. They'll even help you get in touch with the services that can help you most. It's a nice parallel to the support you get in treatment.
Follow up care is another important factor of this program. I find medical care in any field is more effective when there's some form of post discharge check ins with staff. Follow up care is a great way for staff to gauge your progress and for you to voice any concerns you may have. Recovery doesn't end with treatment, so why should care?
