What I think is so unique about this program is the support and assistance it provides for individuals who are affected by medication conditions or are experiencing homelessness. They will monitor you throughout the time you are connected to them to make sure you're staying on top of your medication management and follow-up on appointments. They will also provide advocacy, care coordination assistance and supportive services to your family as well.
