A multidisciplinary treatment team comprising the patient’s doctors and clinicians assesses the patient first and decides on the appropriate level of care. They then create a personalized treatment plan with input from the patients.
The treatment plan outlines issues, solutions, and objectives to help the patient return to the least restrictive setting or the community following discharge. Medication group therapy and case management are typically used in conjunction with traditional therapy. Their therapy is intensely focused and has a short duration.
