About Continuum Behavioral Health and Recovery Services

Continuum Behavioral Health & Recovery Services was formerly known as OnTrack Rogue Valley and is a substance use treatment program in Grants Pass, Oregon offering integrated outpatient care for adolescents and adults across Jackson and Josephine Counties.

The program is built on a trauma informed and culturally responsive philosophy which I noted treats both substance use and co-occurring mental health conditions.

Substance and Tobacco Addiction Treatment

Care typically begins with a comprehensive substance use assessment and screening for mental health and tobacco use to match clients with the right level of support.

From there clients may engage in outpatient or intensive outpatient services and I found they provide referrals to ASAM 3.5 residential treatment when a higher level of care is needed.

Clients can expect evidence based approaches such as cognitive behavioral therapy and motivational interviewing using the Matrix Model, and relapse prevention delivered through counseling.

Telehealth and peer support, case management, naloxone education, and aftercare planning round out the continuum.
