Hawaii residents who can’t afford rehab treatment for addictions now have new options. The state has been selected to participate in a federal program that expands Medicaid rehab coverage for mental health and substance use disorder services at no cost to eligible patients regardless of ability to pay.
The CCBHC Medicaid Demonstration Program
Hawaii has long featured several low-cost and affordable inpatient and outpatient care programs for residents of all backgrounds. To supplement these services, the Aloha State has been one of 10 states chosen to participate in the Certified Community Behavioral Health Clinic (CCBHC) Medicaid Demonstration Program. The program is operated by SAMHSA and other government agencies.
The program establishes certified clinics that meet federal standards for comprehensive care. They’re required to provide 24/7 crisis care alongside outpatient mental health and substance use services.
Critically for uninsured and underinsured individuals, clinics must serve anyone regardless of ability to pay, age or place of residence. That means even people without Medicaid or private insurance can walk through the door.
Qualifying for Free or Low-Cost Treatment in Hawaii
Since CCBHC clinics operate on a no-refusal basis, eligibility requirements have broad standards. Folks can qualify for free or reduced-cost services if they meet the following:
- enrolled in Hawaii’s Medicaid program (Med-QUEST)
- uninsured or underinsured
- unable to afford private rehab centers
- experiencing a mental health or substance use crisis
A partnership between the Hawaii Department of Health and the Department of Human Services’ Med-QUEST Division will administer the program. Med-Quest already runs the state’s Medicaid program.
If you’re already on Medicaid, your coverage should include these services. But for those not enrolled, reaching out to Med-QUEST to check eligibility is a smart first step.
Medicaid Rehab Coverage Matters
For many Hawaii residents, cost remains the single biggest barrier to getting help. Private inpatient rehab can run $10,000–$30,000 or more per month. Medicaid rehab coverage eliminates or dramatically reduces that burden for qualifying individuals.
Since CCBHC’s launch in 2017, the program has expanded to 500+ clinics in nearly all 50 states, plus the District of Columbia and Puerto Rico. Hawaii’s participation brings the state in line with a proven national model for making substance use treatment accessible to everyone, not just those who can afford it.
Governor Josh Green noted the selection “provides a pathway to bring additional federal support to the people who need these services most.”
Accessing These Resources
Two adult mental health clinics in Kahului and Lahaina should become fully certified by January 1, 2027 and serve more than 1,700 Maui residents. Officials expect more locations statewide to follow.
