In addition to providing you with mental health management services, Horizon Wellness Center also offers case management for children and adults with intellectual and developmental disabilities.
Case managers will direct you to appropriate community resources to help you establish doctor referrals, coordination of care, and to assist you in establishing regular follow ups for needed services and support.
