The program addresses drug addiction, serious mental health issues, unemployment, transportation, homelessness, and overall poor health by employing wraparound services like substance use and co-occurring disorder treatment. The program has four phases and lasts for 12 months. All the services at this location work together for an integrated health experience, with primary care services also available.
