About Daviess County Family Health Center
In 2023, Samaritan Center merged with the Family Health Center to become the Daviess County Family Health Center. This accredited provider by The Joint Commission offers primary care alongside behavioral health and addiction services (including medication assisted treatment, or MAT) to deliver whole person care. You’ll find their clinic in Washington, Indiana, not far from scenic East Side Park.
Services here take place in an outpatient setting, but if you require a higher level of care, they can make a referral. Among their specialties are trauma support, medication management, and serious mental illness.
Medicare and Medicaid are accepted here. I’m also impressed that they also offer support for those without insurance.
Chances Program
Those stepping down from inpatient treatment who may still be in need of a higher level of care may find the Chances Program a good fit. It’s an intensive outpatient (IOP) program where you work with a large care team targeting every aspect of your physical and mental health, as well as social workers and other health care professionals. All your concerns, including those relating to food security, shelter, and other aspects of independent living, are addressed as part of care.
Hosted Recovery Meetings
This location also hosts recovery meetings throughout the community. I saw that there are meetings for AA and NA, as well as support groups for celebrating recovery and sobriety.
Amenities
Private drug rehab provides a comfortable, secure environment that allows you to focus on doing the work to get your life back on track. Benefits include a higher staff-to-client ratio, increased one-on-one time with therapists and healthcare providers, private rooms for clients, and customized forms of therapy.
In order to maintain a sense of autonomy, many private rehab facilities offer clients the ability to choose their own private rooms. The privacy and personal space ensure that the recovery process is as comfortable as possible.

Assertive Community Treatment (ACT) is an integrative, community-based care strategy designed to address the needs of persons with severe and/or complex mental illness or behavioral disorders. ACT is typically provided by a multidisciplinary team of medical and mental health care providers, social workers, therapists, and other specialists, including addiction recovery professionals. These services are frequently provided in the home and community to clients in crisis, those who are clinically unstable, and those who are unable or unwilling to travel to a hospital or clinic for in-person treatment.
Levels of Care
Indiana outpatient rehab focuses on providing tools necessary to stay sober. This involves several weekly counseling sessions and involvement in at least one support group, such as NA or AA, for several weeks or months. During that time, you’ll learn how to replace unhealthy habits with healthy ones and prevent relapse.
Dual diagnosis in Indiana treats co-occurring disorders, meaning it addresses both your mental health and recovery needs. A multidisciplinary team of psychiatrists and addiction experts work together to ensure all aspects of your mental and physical health are treated.
While in a partial hospitalization program (PHP), you receive treatment similar to an inpatient program, but you’re able to live at home during non-treatment times. PHPs often serve as a step between inpatient and outpatient drug rehab in Indiana.
