The assistance they provide is specific to your current situation, medical history and your goals. They will also provide updates and plan for your next steps.

You’ll graduate with an individualized continuing care plan. The plan will be different from other individuals as it is dependent on your needs. Your next steps will be arranged. Your next step may be residential treatment, partial hospitalization (PHP), outpatient (OP) treatment, sober living or possible mental health treatment. They can provide referrals if need be.
