About Daybreak
Located in Palmer, Alaska, Daybreak provides case management services for individuals struggling with mental health challenges, and traumatic brain injury, and who are reentering society after prison. If addiction is the primary diagnosis, a client may need to be referred to an alcohol and drug rehab center.
Daybreak provides services that help clients gain independence and advance in their life. They help individuals navigate health benefit issues, identify and work with practitioners and professionals who feel aligned with them, and provide patient advocacy. If transportation is needed, it can be offered. Clients also may need assistance as they settle back into a normal routine after serving time in prison. Daybreak helps them identify their basic goals for their new life and assists them in establishing the appropriate connections to help them meet their most basic needs in life. Daybreak can also help them with referrals to outside agencies that may provide services Daybreak cannot offer.
