About Crossroads Health – Integrated Healthcare Center

Located in Mentor, Ohio, Crossroads Health provides dual diagnosis outpatient treatment to individuals with mental health and substance use disorders. Trauma Focused Therapy and EMDR

Crossroads Health’s trauma informed treatment is delivered through outpatient, intensive outpatient and partial hospitalization programs. Services are designed for adults, adolescents and children meeting the needs of each specific age group.

Teen & Adolescent Program

Adult Program
