About Transformations Wellness Center

The Transformation Wellness Center is located in Klamath Falls, Oregon. They provide adults, who have a substance use disorder with residential treatment, outpatient treatment, and recovery housing. If you are a resident of Oregon and your family household income qualifies, you can use the sliding scale fee. They also accept private pay and the Oregon Health Plan.

The residential facility is a 24-bed, co-ed facility. Here, you will be in a supervised, controlled environment. You will also have medical and emotional support that is available 24/7. Having this type of environment will help you overcome addiction.

If your care requires something less restrictive, then their outpatient substance use disorder treatment may be for you. In this program, you will attend individual and group counseling. There is case management, peer support, and DUI services. The counselors will teach you how to cope without drugs.

The Breaking the Cycle Recovery House is available if you need a safe environment to live in while attending outpatient treatment. You will be in a clean and sober environment. They can house 10 women and 12 men at a time.
