About Threshold Rehabilitation Services

Threshold Rehabilitation Services, Inc. is a CARF-accredited nonprofit in Reading, Pennsylvania. This center provides residential, vocational and mental health support for adults with intellectual and developmental disabilities, serious mental illness, and economic barriers.

Since 1965, TRS has offered person driven care including 24-hour residential homes, a partial hospitalization program (PHP) and employment services tailored to individual goals. Their holistic approach emphasizes community integration and independence to help individuals transition from high acuity settings to meaningful, self directed lives.

Mental Health Residential and Partial Hospitalization Services

TRS specializes in intensive mental health support for adults recovering from serious mental illness through their Community Residential Rehabilitation program and weekday PHP. Residential services provide 24-hour staff support in transitional housing while PHP offers structured group therapy, psychiatric evaluation, medication management and discharge planning as an alternative or step down from inpatient hospitalization.

Treatment uses a bio-psycho-social model with programming in symptom education, life and social skills, grief recovery, depression and anxiety management and recreational therapy. Multidisciplinary teams led by licensed psychiatrists work collaboratively to address root causes, build coping skills and create clear pathways toward independent living and community reintegration.

Intellectual and Developmental Disability Supports

TRS operates residential community homes for one to four individuals with intellectual and developmental disabilities staffed 16 to 24 hours daily. Services include daily living skills training, healthcare coordination, financial management, recreation and interpersonal development all designed to foster autonomy and meaningful community participation.

Getting Started with TRS

TRS serves adults throughout Berks County with in-person services at their Lancaster Avenue location in Reading and satellite sites offering 24-hour residential support and weekday PHP programming. TRS accepts Medicaid and Medicare for psychiatric services with additional insurance options available for other programs.
