The San Fernando Valley Community Mental Health Center is a CARF accredited nonprofit organization in Van Nuys, California. They provide comprehensive mental health services across the San Fernando Valley. They offer a wide range of programs to individuals of all ages including children, adolescents, adults and the elderly.
Specialized Elderly Mental Health Program
One of their key programs is the Older Adult Full Service Partnership (OAFSP). This program provides intensive mental health services and case management to individuals aged 60 and older.
The program is designed to help seniors navigate the challenges that come with aging. Some of these challenges include isolation, depression and cognitive decline.
The Milestone program stabilizes and reintegrates individuals back into society. They help adults who are experiencing homelessness, have been incarcerated or are mentally ill. You’ll receive case management and psychiatric support.
Another program that stands out to me is their adult Prevention and Early Intervention (PEI). This is a short term program with four treatment models. It provides services for adults 60 years and older who may be at risk of substance abuse and mental health issues. Please note this program is designed specifically for those who haven’t received mental health services in the past.

Their client engagement and navigation services are for those who are ready to accept treatment for their substance use disorder. This program is funded through Substance Abuse Prevention and Control by the LA County Department of Public Health.
