About Project Transition Reading

Project Transition is a community-based behavioral health program in Reading, Pennsylvania for adults 18 and older living with serious mental illness and co-occurring substance use disorders.

The program combines a therapeutic community model with apartment-based residential living, daily psychiatric rehabilitation, Dialectical Behavior Therapy and coordinated outpatient supports.

Project Transition's mission centers on hope, normalization and long-term recovery within real-world community settings rather than institutional environments.

Dual Diagnosis Treatment for Adults

Project Transition Reading specializes in supporting adults whose serious mental health conditions are complicated by substance use.

I noticed that members receive individualized treatment plans built around psychiatric rehabilitation, medication management, family engagement and recovery management for co-occurring disorders.

Daily programming includes therapy, group sessions, community meetings and life-skills workshops at nearby recovery centers, with DBT serving as a core evidence-based approach.

Staff are on-site from 7 a.m. to 9 p.m., and members can access 24/7 skills coaching and crisis response between sessions.

Apartment-Based Therapeutic Community Living

Rather than a traditional facility, I noted that members live in furnished apartments or townhomes rented by Project Transition, typically with up to two roommates.

This community-integrated model emphasizes reduced stigma, real-world skill-building and reintegration, supported by WRAP planning, positive behavior supports and coordinated physical healthcare.
