About Orange County Health Care Agcy

Perinatal and Infant Assessment and Coordination Team is an Orange County nurse visitation program known as PACT.

It supports pregnant and parenting women affected by substance use or HIV as well as medically high-risk newborns and their families.

PACT connects mothers and infants to the care, resources and community services they need to build healthier futures during pregnancy and beyond.

Perinatal Substance Use Support and Care Coordination

I appreciate that PACT specializes in supporting pregnant and parenting women with current or past substance use through in-home nurse visitation and psychosocial assessment.

Public Health Nurses provide prenatal and postpartum education, case management and referrals to alcohol and drug treatment programs.

Follow up care continues throughout pregnancy and up to 12 months after delivery.

Sessions focus on health teaching for mothers and babies, coordinated referrals and linkage to prenatal care, pediatric services, family planning, housing and transportation.

This holistic, in-home approach meets families where they are and addresses both medical and social needs.

Infant and Family Health Support

Beyond maternal care I found that PACT provides dedicated nurse visits for medically high-risk newborns and their families.

The team integrates infant health monitoring with maternal support to promote safe, stable environments for the first year of life.

Limited transportation assistance and bus passes help families access essential medical appointments.
