About Momentum for Mental Outpatient

Momentum for Health’ outpatient clinic provides alcohol and drug rehab services in San Jose, California. Operating in coordination with the Santa Clara County Behavioral Health Services Department, this treatment center serves clients over age 18 in Santa Clara County who are active Medi-Cal recipients and have a prior behavioral health diagnosis, including substance use disorder.

Treatment programs are individualized for every client based on their behavioral health history and a personal assessment at the start of their program. Each client has a case manager who supervises their program. Along with individual counseling and group counseling, clients can benefit from medication management, life skills education, psychiatric support, and employment assistance for long-term success in recovery.

All clients must be referred by Santa Clara County to enroll. This facility accepts a variety of major insurance providers including Beacon, Magellan, Blue Cross Blue Shield, Anthem, Aetna, Cigna, Optum, Kaiser Permanente, and MHN. Confirm your benefits with your insurance provider because coverage and out-of-network benefits will vary.
