Momentum for Health Eastside Outpatient (MHE) is a facility in San Jose, California. With a long history of providing care dating back to the 1950s, MHE has experience with evolving to best serve its community’s health. History of MHE and Adjusting Care Based on Your Needs

Just like people, communities’ needs change. I think that MHE is cognizant and intuitive to where facilities or treatment need adjustment or growth. You must be enrolled in Medi-Cal and live in Santa Clara County to be eligible.

One example of this is how they saw a shortcoming in overall well-being care. Since mental health and substance abuse are not always independent issues, MHE grew to care for co-occurring conditions. MHE grew from hospitals to community care and mental health to overall well-being, including substance abuse treatment.

MHE has five main priorities that all center around evaluation and growth, resulting in continuous improvement. While honing process and treatment, MHS also commits itself to diversity and inclusion, both in staff and in delivering services.

Outpatient Care & Housing

While MHE offers residential housing services, this location is for outpatient care, specifically for adults, offering a variety of services such as medication management and counseling. Some outpatient programs are also for patients living in supportive housing programs.

With an approach that caters to you as a whole, MHE addresses your mental, emotional, physical, and social well-being.

I believe that numbers can be important when considering care or services. With individual care and attention, MHE serves about 1500 clients across its four locations, showing that they have the strength to care for many patients.
