Momentum for Health's Downtown Outpatient clinic is a behavioral health and alcohol and drug rehab program connected to the Santa Clara County Behavioral Health Services Department. This outpatient program serves Santa Clara County residents ages 18 and older who are active Medi-Cal recipients and have a behavioral health diagnosis. They're out in San Jose, California.
