About Fellowship Health Resources

The OSOP in Cranston is a community outreach program that combines Assertive Community Training and a Health Home Service Model.
The program provides comprehensive care coordination with specialized clinical care for adults with persistent and severe mental illness. Eligible adults receive supportive services, including care management and coordination with staff arranging psychiatric, medical, and financial services.
The staff helps clients identify goals and provide assistance in finding and maintaining housing. They assist with transitional care, health promotion, family and individual support services, and referral to support services within the community.
The OSOP focuses on an individual’s rehabilitation so the client can manage his/her substance use and co occurring mental health disorders. Providers work to increase a client’s independence by improving the client’s abilities in regard to activities of daily living.

Peer support services are included in the adult programming. Peer support is offered by individuals who have been through the recovery process and remain engaged. Peer supporters help through group therapy, individual meetings, and wellness training.
I think clients will appreciate the traditional outpatient mental health services too. These include therapy and psychiatry. Intake includes a comprehensive assessment that opens access to all the outpatient services. Additional services are testing, psychiatric medication management, and group and individual therapy.
