About RiseWell Community Services
Contact Federation of Organizations Patchogue Community Center is a social service organization located in Patchogue, New York. The Federation provides many different health services including behavioral health care. This particular facility offers a program called P.R.O.S that is ideal for people who have both a mental health condition and substance use disorder concurrently (dual diagnosis).
How The Program Works
Recovery Concepts -Personalized Recovery Oriented Services (P.R.O.S.) program incorporates wellness training, personalized therapy, and intensive support services.
You can get treatment at the community center, but the therapists also provide treatment at different locations in the community.
Intensive Rehabilitation
The trained counselors can help people with mental health issues and/ or SUD achieve both personal and professional goals. They have different workshops that show you how sobriety and mental health care can help you obtain housing and employment.
Continuous Support
One thing I like about this program is that the counselors help people retain employment and housing. It has been my experience that many rehabs help people get jobs and housing, but not enough work to help people maintain those things in the long term.
