Downtown Family Health Center at Connections is a community health center in San Diego providing integrated primary care, behavioral health and substance use disorder services for individuals experiencing or at risk of homelessness.

The center offers a comprehensive one stop model that includes medical care, counseling, psychiatry, case management and recovery support services. Located within the Connections Housing campus, we like that the clinic connects patients to wraparound care that addresses health, housing and stability in 1 accessible setting.

Integrated Substance Use Disorder and Behavioral Health Services

Downtown Family Health Center at Connections specializes in treating substance use disorders alongside mental health and medical needs for San Diego's most vulnerable populations.
