About Contra Costa TAY Program
Contra Costa TAY Program, part of Fred Finch Youth & Family Services, is a community based behavioral health provider serving transition age youth ages 16 to 25 in central and west Contra Costa County, California.
The program supports young adults facing serious mental health challenges and trauma, substance use and housing instability through therapy, case management and medication management.
Its recovery oriented model meets young people where they are, blending clinical care with real world support for those often underserved by traditional providers.
Dual Diagnosis for Young Adults
The Contra Costa Transition Aged Youth program specializes in helping young adults navigate mental health and co-occurring substance use concerns during a pivotal life stage.
Services include rehab and dual diagnosis support, crisis intervention and suicide prevention for youth with complex needs.
Treatment draws on evidence based approaches like cognitive behavioral therapy, dialectical behavior therapy, trauma therapy and behavior modification.
Sessions focus on building coping skills, addressing underlying trauma and fostering long term stability rather than short term fixes.
Housing, Education, and Stability
Beyond therapy, CCTAY integrates housing stabilization, educational support and supported employment to help young adults build self sufficiency.
Peer support, psychosocial rehab and vocational services round out a whole person approach.
