About Passages Transitional Housing Program

Communicare Passages Transitional Housing provides residential and outpatient treatment for substance use disorders and co-occurring mental health conditions in Elizabethtown, Kentucky. They accept Medicaid, Medicare, and payments on a sliding scale. Services are available for the deaf and hard of hearing, and they offer specialized programs for adult female clients, court referred clients, and young adults aged 18 to 25.

MAT for Alcohol Use Disorder

While they do accept patients with preexisting medication assisted treatment (MAT) prescriptions, they only offer MAT for alcohol use disorder onsite. Their comprehensive continuum of care includes mental health services, case management, social skills development, and transportation assistance. They also work with those needing help with obtaining housing or other community services. Residential beds are available for the children of current patients which I think is a tremendous help for families who need healing.

Services Under the Matrix Model

Based on the Matrix Model, their program provides a variety of modalities including cognitive behavioral therapy (CBT), 12 step facilitation, and motivational interviewing. They also offer anger management services, relapse prevention support, and education on substance use and HIV/AIDS. Family counseling and peer support play a large role in the program. I was glad to see that telemedicine is available for those who wish to receive outpatient services from the comfort of their own homes, and it makes treatment accessible for people who don’t have reliable transportation or childcare.
